Insecurity Insight identified 735 incidents of violence against or obstruction of access to health care in Sudan since fighting started between the Sudan Armed Forces (SAF) and Rapid Support Forces (RSF) in April 2023 and 31 March 2026. At least 195 health workers were killed and 126 arrested, while health facilities were reportedly damaged on 205 occasions. Most of these 735 incidents were attributed to the RSF.

Food security is a major concern in Sudan. Between February and May 2026, 19.1 million people are projected to experience high levels of acute food insecurity (IPC Phase 3 or above). Insecurity Insight’s monitoring of violent events showed an alarmingly high rate of conflict incidents affecting the food system, with violence at markets as well as incidents of cattle looting and killing and attacks on civilians growing food, selling or buying food reported in high numbers. Health needs are also high throughout the country. As of March 2026, two out of three people in Sudan no longer have access to essential health services and 80% of hospitals are out of service in affected areas. The lack of protection of vital civilian infrastructure is a driver of escalating humanitarian needs across Sudan.
This brief identifies trends and recurring patterns, highlights how the conduct of hostilities contributes to creating humanitarian needs, and provides insights into anticipating them. It also provides a list of actionable and practical recommendations in service of sustaining international action to mitigate harm.

The publicly reported incidents below are not a complete nor a representative list of all incidents that affected the provision of health care between 01-14 April 2026. 01 April 2026: In Kosti/Rabak town, Al Jabalian district, White Nile state, the Central Medicine Warehouse was hit and destroyed by a drone, injuring a pharmacist and two warehouse workers. Sources: Sudan Tribune, UN News and WHO
01 April 2026: In Khor Ramla area, Gharb Jabal Marrah locality, Central Darfur state, the tyres of an International Medical Corps vehicle were reportedly shot at by an unidentified armed group, and the crew’s belongings looted. Source: ACLED1
02 April 2026: In Al Nadhir neighbourhood, Al-Daein city, East Darfur state, the Family Hospital was raided and looted by a force affiliated with the RSF members, who assaulted and expelled health workers and patients. 02 April 2026: In Al Jabalain city, White Nile state, Al-Jabalain Hospital was hit by two RSF drone strikes, damaging the operating theatre and the maternity ward, killing at least 12 people, including ten health workers, two of whom were doctors. 09 April 2026: In An Nuhud town and district, West Kordofan state, a hospital was reportedly hit by an SAF drone strike, killing and injuring an unspecified number of people. Source: ACLED1
